How To Refer Patients

You have identified a patient that needs palliative care, telehealth, or would like us to visit in-home for evaluation.
Here’s what comes next:

Send us a referral form via fax or email. Please include a face sheet, history, physical, and any clinical notes you may feel relevant. Note, for your convenience we have included a referral form, but feel free to use your own!

Confirm your patient is covered by Molina, Blue Shield of California, or Promise.

Fill in the referral form (ours or yours).

Within 30 minutes of receipt of the referral, someone from our team will confirm.

After this, we will follow our intake steps which include scheduling an evaluation, and if they qualify, we will admit them to the program.

Once your patient has been admitted, we will confirm with you, and continue to provide updates and reporting.
