Kara Health Authorization Triggers :

  • Difficultly in performing activities of daily living, ADLs
  • Unrelieved pain
  • Confusion
  • Shortness of breath or fatigue at minimal exertion • Multiple emergency department visits or hospitalizations
  • 10% weight loss in a 6 month period
  • Recurrent falls with injury
  • Decreased tolerance and physical activity

Primary Diagnosis is One of the Following :

  • Cancer (Stage 3, 4, or 5)
  • Heart Disease
  • HIV
  • Liver Disease
  • Renal Disease
  • Pulmonary Disease (Chronic Obstructive Pulmonary Disease,
    Interstitial Lung Disease, Pulmonary Fibrosis)
  • Neurological Disease (ALS, Alzheimer’s, Brain Tumor, Huntington’s, Lewy Body Dementia, Multiple Sclerosis, Parkinsons, Stroke)
