Our services are all available in-home or via telehealth

Conduct in-home assessment and evaluation by NP or Physician

Home visits provided by interdisciplinary team of physicians, nurses, clinical therapists, dieticians, and chaplains/spiritual guides

24-7 nurse line and nurse availability to perform crisis interventions as needed

Disease-specific palliative care case managers are assigned to each patient

Case managers maintain high level of communication with patient’s primary care physician, specialist, and other ancillary service providers

Provide in-home medication reconciliation

Discuss patient and family goals of care, advance care planning and complete POLST

Coordinate transition to other care settings such as home health, home infusion, private caregiver and/or other resources as needed

Patient concierge will work with health plan to expedite authorizations of medications, DME, and other services such as transportation
